Complete the application below to enroll your child in The Gifted Enrichment Program. Have your child's school and emergency contact information handy before you start.
If you are registering more than one child, you must complete this registration form separately for EACH child.
CSC MOST Sliding Fee Scale Requirement
As part of enrollment in the GIFT-Ed Enrichment Afterschool Program, families are required to complete the CSC Sliding Fee Scale Assessment. Household size and annual gross household income are used to determine whether a weekly program copayment applies.
GIFT-Ed staff will determine your weekly copayment using the current CSC MOST Sliding Fee Scale. You may be asked to provide supporting income documentation.
Financial Hardship: Participation in the program will not be denied because of an inability to pay. If your family is assessed a weekly copayment but is experiencing financial hardship, you may request a waiver. Additional documentation explaining the hardship may be required.
